CONSCIOUS RELATIONSHIP TRAINING
RELEASE OF LIABILITY, WAIVER OF CLAIMS AND ASSUMPTION OF RISKS 

It is important that you read the following carefully. By agreeing, you consent to waive certain legal rights including the right to sue. 

1. I acknowledge that I have voluntarily registered to participate in a Conscious Relationship Training course or workshop (“CRT Course”), organized by Stephanie Hunter, Georgianna Lee, and/or their assistants and volunteers (the “Trainers”).
2. I acknowledge that all payments made for this training are non-refundable, except in the event of an unforeseen, serious medical condition, or such other bona fide unforeseen emergency that prevents you from being physically unable to attend the training. 
3. I understand and acknowledge that the CRT Course is intended to assist the development of my self-awareness. The training is non-religious and non-political.
4. I am aware that participating in the CRT Course may involve the following:

• (a)    group discussions and workshop exercises that may include some physical contact and risk of injury. I understand that participants will have the option to consent or choose to not participate in any activity at all times; and,
• (b)    psychological and spiritual training and that I may experience intense and unusual psychological, spiritual and/or physical states of mind and body arising from my participation.
• I am voluntarily participating in the CRT Course with full knowledge of the risks involved and hereby agree to accept any and all risks of harm that may result. 

5. I affirm that I am medically, physically, emotionally and in all respects fit and able to participate in these meetings and activities. I will not hold the Trainers responsible for any injuries suffered by me caused in whole or in part by any physical impairment of mine not fully disclosed to the Trainers in writing.
6. During the meetings and activities, I will follow all instructions given to me by the Trainers as to how to perform any workshop exercise. I understand that any deviation from these instructions by me shall be at my own risk. I will not hold the Trainers responsible for any injuries suffered by me caused in whole or in part by my failure to follow instructions.
7. If I experience pain or discomfort I will notify the Trainers. I will not hold the Trainers responsible for any pain or discomfort I experience during or after the event.
8. I acknowledge that none of the Trainers are licensed mental health professionals and that CRT Course meetings and activities are not intended as medical therapy, nor are they intended to supersede or replace any advice given by any medical or psychiatric health care worker. I assume full responsibility for my experience in the CRT Course.
9. I hereby authorize the Trainers to publish photographs taken of me during the event and my name and likeness for use in CRT Course print, online and video-based marketing materials. I hereby release and hold harmless the Trainers from any reasonable expectation of privacy or confidentiality associated 
with the images specified above. I further acknowledge that my participation is voluntary and that I will not receive financial compensation of any type associated with the taking or publication of these photographs or participation in marketing materials or other CRT Course publications. I acknowledge and agree that publication of said photos confers no rights of ownership or royalties whatsoever. I hereby release the Trainers, its contractors, its employees, and any third parties involved in the creation or publication or marketing materials, from liability for any claims by me or any third party in connection with my participation. 
10. I agree that I, my assignees, heirs, guardians, and legal representatives will not make a claim against or sue the Trainers, their affiliates, employees, agents, or volunteers or any of their affiliated organizations for injury or damage resulting from acts, however caused.
11. I release the Trainers from all actions, claims or demands that I, my assignees, heirs, guardians, and legal representatives now have or may hereafter have for injury, damage, or death resulting from, or in any way connected with my participation in the CRT, including loss that may be caused by the negligence of the released party.
12. I shall not now or at any time in the future bring any legal action against the Trainers and agree that this waiver is binding on me, my heirs, my spouse, my children, my legal representatives, my successors and my assignees.
13. If any part of this waiver is unlawful, void or unenforceable, then that part shall be considered severed, and will not affect the validity and enforceability of the remaining clauses.
14. I have carefully read this agreement and fully understand its contents. I am aware that this is a release of liability and a contract between myself and the Trainers and agree to it of my own free will.
